Attorney Requests

The fastest method for requesting billing records is to send the request online via the Chart Swap website. After logging in, search for our company name in Florida. Fill in the patient details and attach relevant documents before submitting the request. Our billing department typically answers uploaded requests within a week of receiving payment.

ALL of our billing record requests go through Chart Swap. If you email, fax, hand deliver, or mail a records request or subpoena to us, it will be input into Chart Swap.

Our office will only be able to supply billing records. The patient’s medical records must be requested from the hospital directly.
