Visiting our Clinic
We are happy to accept your referring physician or primary care physician’s referral to our clinic to be evaluated by one of our board-certified interventional radiologists or neurointerventional surgeons.
Before you call, please gather these items so that our patient care coordinator may help:
- Your Referring Physician’s name and contact information
- Your Primary Care Physician’s (PCP) name and contact information
- The script (Rx) from your physician that states the procedure you need
- Your insurance card(s)
To schedule an appointment with our clinic staff, please call our scheduling department at 305-712-7229 and push option 1. We are open Monday through Friday, 9AM to 4PM. We are closed for lunch from 12-1PM.
Your physician will need to send us these documents prior to your clinic visit with us:
- Recent Progress Note (H&P) showing medical necessity for the procedure requested
- Legible Script (Rx) signed by your physician with the procedure and diagnosis
- Demographic facesheet
- Recent lab work (if available)
- Recent relevant radiology reports (if available)
Patients with HMO Insurance:
If you have an HMO insurance plan, we will need to contact your Primary Care Physician (PCP) to request a referral for an outpatient office visit before seeing you in our clinic. After your clinic visit with us, we will need to send your PCP a referral request for the interventional radiology procedure.
Before your clinic visit:
If you have had imaging done (such as a CT scan, MRI, or PET/CT) at another radiology office or at a hospital other than one of our partner hospitals, please request a CD of the images. If you had scans done at one of our partner hospitals, then we have those images already.
If you have time and the ability, we would greatly appreciate you dropping the CD off at one of our clinics (Aventura or Kendall offices). You may also mail it to our office (IntelliRad Imaging, 11760 SW 40th St, Ste 703, Miami, FL 33175). We will then be able to load your images to our computers so that our physicians may review the images prior to seeing you in our clinic.
If you do not have the ability to drop off the CD or mail it to our office, you will need to bring the CD to our office on the day of your clinic visit. Bringing the CD on the day of your visit will likely cause you to wait longer as our physician will need to review the images on the CD before seeing you.
On the day of your clinic visit with us, please bring these items with you:
- Your reading glasses
- Your drivers license or ID card
- Your insurance card(s) – both primary and second insurances (if applicable)
- CD of images from recent radiology scans if you didn’t already drop them off
After your clinic visit:
Our clinic staff will come up with a treatment plan to suit your clinical needs. We will coordinate with your physician’s office to get the authorization approved if your insurance requires one for your procedure. After we receive the approved authorization, we will call you to schedule your procedure.
Before your procedure at one of our partner hospitals:
Lab work: all of our partner hospitals require that all sedation patients have recent bloodwork done within the 14 days prior to your procedure to ensure that you are healthy enough for sedation.
We will order the below labs for you when we see you in our clinic:
- Lab request – CBC
- Lab request – PT/PTT/INR
- Lab request – CMP
- Lab request – COVID-19 PCR test
You will need to visit a LabCorp or Quest office at least a week prior to your procedure date at the hospital. This will allow the lab enough time to send the results to our office.
If you prefer to use the hospital’s lab or if you do not have time to visit an outside lab, you may visit the hospital’s lab at least two (2) days prior to your procedure date.
If you do not have time to get lab work done prior to your procedure date, you may show up to the hospital two (2) hours before your scheduled appointment time to have lab work done. Please be aware that this could cause you to wait longer for your procedure. We recommend getting your lab work done before your procedure date.
Pre-Registering at the hospital:
While you are at the hospital getting your lab work done, please visit the Registration Department to pre-register for your procedure. This will speed up your day-of hospital visit.

After your appointment:
After your procedure, we will send the radiology report and your pathology results (if applicable) to your referring physician’s office.
If necessary, we may also ask you to come to our clinic for a follow-up appointment.
Pre-Operative Medical Clearance for General Anesthesia:
Most of our interventional radiology or neurointerventional surgery procedures only require conscious sedation which does not need to be administered by an anesthesiologist. If you or your procedure require general anesthesia, then the hospital will require a medical clearance form from your primary care physician.
Medical clearance typically requires the below exams and lab work. Your PCP will receive the results and then give you a physical exam in their office.
- Lab request – CBC
- Lab request – PT/PTT/INR
- Lab request – CMP
- Lab request – Urinalysis
- Lab request – COVID-19 PCR test
- EKG
- Chest X-Ray
- Progress Note (H&P) with medical clearance within 15 days of procedure date
If your PCP agrees that you are physically capable of undergoing general anesthesia, then they will provide you with a medical clearance form. That form must be sent to our office to that we may provide it to the hospital when we schedule your procedure appointment.
